Freesia Memory Assessment Service (MAS) - Private Clinical Assessment


Overview:

Our memory assessments can be conducted in-person, via video, or by phone. Being an International service , we have designed our services to serve different client groups. Remote assessments may require family or caregiver involvement to address any limitations. Comprehensive information from both the patient and informant is essential for accurate diagnosis.



Pre-Assessment Considerations

Before the assessment, we explore key symptoms such as sudden onset, rapid dementia progression, unusual headaches, and neurological symptoms. If these are present, immediate action may be needed.

Patient and Informant Interviews

1. Patient Concerns: What brings the patient to see us? What are their main worries?

2. Family Input: Family or caregiver insights of the patient's daily functioning, behavior, and cognitive changes are crucial.



Symptom Review

• Memory: Issues with recent memory, recall, repetition

• Language: Word-finding difficulties, speech clarity

• Visuo-spatial: Orientation, recognizing faces, direction sense

• Executive Skills: Planning, judgment, reasoning

• Behavior/Personality Changes: Attitude shifts, behavioral symptoms like agitation or hallucinations



Functional Assessment

1. Daily Living Activities: Evaluate self-care tasks like bathing, dressing, and eating.

2. Instrumental ADLs: Assess complex activities like managing finances, shopping, and medication.

3. Technology Use: Ability to use phones or devices.

4. Leisure: Maintenance of hobbies and interests.
